PD Registration Form
To register for the listed workshops, complete and
fax the Registration Form to Farhat Khan at 773/534-3981. Workshop
attendees will receive CPDU credits. For more information and date
confirmation, please contact Mrs. Khan at 773/534-3987. The
Registration Fee may be paid on the day of the workshop. Checks are
to be made payable to the Office of Language and Cultural Education.
